This role is based onsite, primarily focusing on providing home care services directly to clients within their residences. Key Responsibilities

The Physical Therapist will:

• Develop and implement individualized care plans that target the prevention of disability, alleviation of pain, improvement of muscle function, and preservation of the client's highest possible level of performance.
• Oversee and mentor Physical Therapy Assistants to ensure the seamless execution of care plans.
• Utilize diagnostic tools to evaluate patients' functional abilities, conditions, and environments, making tailored recommendations to foster independence.
• Administer therapeutic exercises, traction, and manual techniques while instructing and motivating clients in non-manual activities.
• Evaluate, fit, and recommend prosthetic and orthotic devices, liaising with physicians for any necessary modifications.
• Maintain accurate clinical documentation, including initial evaluations, progress notes, and discharge summaries, in line with regulatory requirements.
• Participate actively in interdisciplinary case conferences to strategize comprehensive treatment approaches for clients.
• Provide education and support to clients and their families about the physical therapy process, equipment usage, and home care strategies.
• Be accessible to Physical Therapy Assistants during treatment sessions, ensuring quality oversight as mandated by state regulations.
• Engage with relevant community resources to enhance client care and support.

Patient Population / Client Focus

Our typical patient demographic includes individuals recovering from surgery, those with chronic conditions, and elderly clients requiring rehabilitation support. The Physical Therapist will work closely with a diverse range of patients, tailoring interventions to suit their unique needs and circumstances, all while fostering a warm and supportive environment.

Qualifications

We are looking for candidates who possess the following qualifications:

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A current and valid PT license in the state of Virginia.
