About the Role The Medical Referral Specialist assists patients and expedites the process of referrals to internal and external healthcare specialists. This position helps patients navigate the referral process, coordinates necessary medical records and information, and works with patients, providers, and external organizations to support timely access to care. Key Responsibilities Coordinate and manage internal and external medical referrals, including scheduling, verifying demographics, and tracking referral progress Serve as a point of contact for patients with questions or concerns regarding internal and external medical referrals Coordinate with Medical Records to ensure required records, imaging, and procedural results are available to external providers before patient appointments Assemble and provide appropriate clinical information and records to specialists according to referral guidelines Communicate with referral organizations, providers, and patients to ensure necessary information is available and referrals are addressed promptly Inform patients of upcoming specialist appointments and provide guidance regarding required documentation Assist patients with navigating healthcare system barriers, including interpreter services, transportation, and scheduling needs Enter orders and patient demographics into external systems and portals as needed Maintain accurate referral tracking and documentation and follow up on missed appointments according to SCHS policy Maintain referral contact information and tracking resources Collaborate with the Cancer Care Outreach Coordinator on mammogram and diagnostic colonoscopy referrals Cross-train and support medical records functions in collaboration with the Medical Records Specialist Participate in staff meetings, committees, and other organizational activities Complete other duties as assigned by leadership What You Bring Confidence, professional judgment, and grace under pressure Excellent verbal and written communication skills Strong time management, organization, attention to detail, and prioritization skills Ability to work independently and as part of a collaborative team Flexibility and ability to respond effectively to an evolving workload Ability to build rapport with patients from diverse cultural and socioeconomic backgrounds Commitment to the SCHS mission and staff values Qualifications High school diploma. Medical Assistant experience or knowledge of medical termination desired. PPX (lattice pro entry) experience a plus. Bilingual (English/Spanish) preferred. #J-18808-Ljbffr
