Job Description

We are seeking a dedicated Acute Care Physical Therapist to provide high-quality patient care in a dynamic and challenging environment. The position requires strong clinical skills and the ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Conduct comprehensive evaluations of patients’ physical abilities and needs.
• Develop and implement individualized treatment plans based on patient assessments.
• Monitor patient progress and adjust treatment plans as required to meet recovery goals.
• Educate patients and their families about treatment plans and home exercise programs.
• Collaborate with healthcare team members to optimize patient care outcomes.

Required Experience / Certifications / Licensure

• Licensed Physical Therapist (PT) in accordance with state requirements.
• Minimum of 1 year of experience in acute care settings preferred.
• Familiarity with Epic charting system is a plus.
