Home-Based RN Case Manager — Palliative Care & Coordination

Chapters Health System in Lanham, MD seeks an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide clinical, palliative and supportive care to help patients remain at home when possible. The role requires an active RN license, at least 1 year of nursing experience (hospice or hospital preferred), and a valid driver’s license with reliable transportation.
