Volunteer AmeriCorps Patient Health Navigator

Position Summary AmeriCorps Member – Patient Health Navigator Program. The role supports patients by facilitating access to medical care, providing education, and coordinating community resources. This is a full‑time volunteer position with a modest living allowance stipend; eligibility for a maximum education award upon completion. Responsibilities Schedule appointments and navigate patients through health system barriers. Initiate communication with patients and assist with referrals to address full spectrum of patient needs. Participate in mandatory trainings, service projects and team‑building activities throughout the 10‑11 month term. Create informational materials, posters, handouts, recipe guides, and resource binders for SeaMar sites. Develop a community resource guide for patients and the AmeriCorps‑SeaMar group. Review electronic health record charts and patient lists to identify patients needing health screenings and services; schedule appropriate appointments. Prepare mail outs with relevant materials and patient identifiers. Collaborate with SeaMar medical clinic staff, quality‑improvement staff, and other departments to identify patients for screenings, inform care teams, and reach out to these patients. Help roll‑out preventive health screening and recall efforts to patients. Participate in a site health project that includes community outreach, health interventions, and events on topics such as smoking cessation, diabetes education, nutrition, healthy lifestyles, stress reduction, chronic‑condition management, and adolescent health. Pro‑actively increase public awareness of health‑coverage enrollment opportunities by planning and conducting outreach activities in the community, at both SeaMar‑sponsored and non‑SeaMar‑sponsored events. Assist in developing and distributing informational/promotional materials, and establish and maintain referral relationships with local organizations, agencies, institutions, and businesses as directed by the AmeriCorps Department. Qualifications Bilingual Spanish strongly preferred. Reliable transportation required. Benefits Stipend up to $20,400 prorated over a 1,700‑hour service term ending August2027; additional living‑allowance stipend. Maximum $7,395 education award upon completion.
