This is an exciting opportunity for an experienced LVN who enjoys coordinating care, working closely with residents and families, and helping patients achieve the best possible outcomes throughout their stay.As our LVN Case Manager, you will serve as a key liaison between residents, families, healthcare providers, therapy teams, insurance payers, and community resources to ensure exceptional care delivery and smooth transitions throughout the continuum of care.If you are passionate about resident advocacy, care coordination, and being part of a supportive and collaborative team, we'd love to meet you!Compensation & BenefitsPay$40.00 - $44.00 per hour DOEBenefits IncludeMedical, Dental & Vision Insurance401(k)Paid Time OffEmployee Assistance ProgramContinuing Education OpportunitiesSupportive Leadership TeamCareer Growth OpportunitiesPositive Team-Oriented Work EnvironmentPosition SummaryThe LVN Case Manager utilizes clinical expertise and critical thinking skills to coordinate resident care, communicate with families and providers, manage care plans, oversee admissions and discharges, and ensure residents receive high-quality, cost-effective services.This position plays a vital role in promoting positive clinical outcomes, resident satisfaction, and successful care transitions.Essential Duties & ResponsibilitiesCoordinate and communicate resident plans of care with residents, families, physicians, therapists, and nursing staff.Participate in daily interdisciplinary meetings and care conferences.Educate residents and families regarding treatment plans, therapy services, care goals, and discharge planning.Monitor resident status and identify changing needs.Perform clinical assessments and care coordination activities.Coordinate admissions, transfers, discharges, and referrals.Collaborate with insurance providers and third-party payers regarding authorization and coverage requirements.Conduct utilization review activities to ensure timely and cost-effective care.Maintain accurate and timely documentation.Promote effective communication among all members of the healthcare team.Assist with discharge planning and transition of care services.Build positive relationships with residents, families, physicians, and community partners.Utilize sound clinical judgment in a fast-paced healthcare environment.QualificationsRequiredCurrent California LVN (Licensed Vocational Nurse) license in good standing.Minimum 2 years of clinical nursing experience preferred.Knowledge of Medicare, Medi-Cal, Medicaid, and insurance processes.Strong organizational, communication, and care coordination skills.Ability to work independently and prioritize multiple responsibilities.Intermediate computer skills.PreferredSkilled Nursing Facility (SNF) or Post-Acute Care experience.Experience with care coordination or case management.PCC (PointClickCare) experience.Certified Case Manager (CCM) credential.RN license or Bachelor's Degree in Nursing or Social Work.Why McKinley Park Care Center?At McKinley Park Care Center, we believe our employees are our greatest asset.
