Remote IRIS Care Navigator & Community Support Partner

Molina Healthcare Inc

Molina Healthcare is seeking an IRIS Consultant to support Wisconsin’s IRIS program remotely. You will partner with participants in their home and community, visiting in person at least four times yearly, and help them achieve long‑term care goals within their IRIS budget.

As part of a collaborative team, you will educate participants about their rights, coordinate with providers, and document progress in the IRIS data system while maintaining HIPAA compliance and confidentiality.
