IRIS Care Navigator — Remote & Relationship Builder

Molina Healthcare Inc

Molina Healthcare Inc. is seeking an IRIS Consultant to support Wisconsin participants remotely, with in-person visits to homes and communities. You will help clients set and reach long-term goals, navigate IRIS, and coordinate with local resources.

Responsibilities include documenting activities in WISITs, developing individualized plans, and guiding participants on budget usage while maintaining confidentiality and HIPAA compliance. Equality and diversity are prioritized.
