We will be offering a $25 per hour incentive for night shifts confirmed for providers. All day shifts will stay at our contracted rates. Please present the provider at the contracted rates and we will ensure this is tied to confirmed providers confirmation agreements. • Provider Min # Shifts Required: specific shifts will vary • Guaranteed Hours per Day: 8 • Hours Worked for OT to apply: 12 per day • Rates to be Filled Out on Bid: Clinic : Regular, Orientation, Overtime, Holiday ________________________________________________________________ CREDENTIALING • Current CV (must be uploaded to bid) • License: Active CA license • DEA Required: Yes • Boards: BC or BE within 3-5 years of residency • Skills/Certs: • Other: ABEM/ABEOM certified (Will not accept ABIM/ABFM) Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. _______________________________________________________________________ ADDITIONAL DETAILS: Average number of patients in 24hrs is 110. Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement): • NOT reimbursed: o Mileage from provider’s home to departing airport, as well as from airport to home upon return. o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return. • WILL be reimbursed: o Airport parking at provider’s departing airport if provider drives themselves to the airport. o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed. o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return tr
