Registered Nurse

The Registered Nurse provides general health counseling, education, and guidance to clients utilizing our Nurse Advice Line and triage services.

Essential Functions:

• Conducts individualized assessments and develops a care plan designed to ensure the achievement of established goals.
• Completes and maintains the patient's medical record, including all relevant health information.
• Collects and reviews laboratory results, medication information, and information from the patient's primary care provider.
• Provides health-related education and coaching in accordance with established clinical guidelines, laws, and regulations.
• Promotes patient behavior modification through coaching and the use of motivational interviewing techniques.
• Refers patients to specialized services based on their individual needs, including, but not limited to, mental health, nutritional assessment, medical therapy management, smoking cessation, and other appropriate resources.
• Documents new patient information, assessments, and interventions in the designated software system.
• Maintains effective communication and coordination among Program team members, health plans, and healthcare professionals.
• Participates in educational meetings and case discussions.
• Ensures the confidentiality of patient information and compliance with all applicable state and federal regulations governing Health Call Centers, including HIPAA, as well as internal policies regarding the disposal of confidential information.
• Immediately reports any exposure of Protected Health Information (PHI) to the supervisor.
• Completes all required core and functional training and complies with procedures established by applicable regulatory agencies, contractual requirements, and certification standards, including, but not limited to, URAC requirements.
• Performs other related duties as assigned.

Requirements:

• Associate Degree in Nursing with five (5) years of clinical experience and/or Bachelor of Science in Nursing (BSN) with three (3) years of clinical experience.
• Required Credentials: Active and unrestricted Puerto Rico Registered Nurse (RN) license, including current registration and professional membership/collegiate requirements.
• Advanced knowledge of computer applications and/or Microsoft Office.
• Strong customer service orientation and commitment to providing excellent service.
• Excellent bilingual verbal and written communication skills in Spanish and English preferred.
• Fast and accurate documentation skills, along with the ability to efficiently navigate the Internet and computer systems.
• Availability to work rotating shifts, including weekends and holidays, within a 24/7 Monday-through-Sunday operation. Shift differential pay is provided, as applicable.
