Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ year of outpatient clinical experience; CPR certification; active applicable license for Indiana; no new graduate required. Additional Information: Completes comprehensive patient evaluations, develops and modifies treatment plans, ensures documentation meets regulatory and organizational standards, and participates in student programs under supervision.
