Behavioral Health Specialist Requires LCSW or LPC or LMFT

Title: Behavioral Health Specialist – LCSW, LPC or LMFT Company: Oak Street Health The Behavioral Health Specialist provides care‑management services and evidence‑based short‑term psychotherapy to patients with behavioral health needs. The specialist is a key resource for Care Teams across a region of Oak Street Health primary clinics, coordinating and supporting behavioral healthcare in collaboration with Care Teams and a psychiatric consultant. Core Responsibilities Collaborate with primary care teams to identify and refer patients to the Oak Street Behavioral Health Program. Screen and assess patients for common mental health and substance use disorders. Develop care plans and facilitate referrals to Oak Street’s psychiatric consultant and/or external resources. Provide short‑term counseling and evidence‑based treatments as indicated. Coordinate care with psychiatric consultants, including warm handoffs for telehealth visits and collaboration on care plans. Build relationships with primary care teams and communicate changes in behavioral health care plans consistently. Maintain accurate and timely documentation of patient encounters and counseling sessions in Oak Street’s electronic medical record. Participate in regional and organizational activities to advance Oak Street’s Behavioral Health Program through workshops, feedback sessions, and surveys. Possibly cover multiple clinics, depending on clinic maturity and panel sizes. Perform other duties as assigned. Qualifications Master’s Degree in mental health counseling/healing arts (Social Work, Counseling, Marriage and Family Therapy, etc.) Highest clinical level of license (or local equivalent) Experience with screening for common mental health and/or substance use disorders Experience with assessment and treatment planning for common mental health and/or substance use disorders Familiarity with brief, structured intervention techniques such as Motivational Interviewing, Behavioral Activation, Problem Solving, and Cognitive Behavioral Therapy US work authorization Proficiency with brief, structured intervention techniques (preferred) Working knowledge of differential diagnoses of common mental health and/or substance use disorders (preferred) Proficient in PC skills (preferred) Pay Range: $60,522.00 – $129,615.00 (base hourly rate or base annual full‑time salary, dependent on experience, education, geography and other factors) Benefits include medical, dental, vision coverage, paid time off, retirement savings options, wellness programs, and other resources, based on eligibility.
