The Nurse Practitioner (NP) will provide comprehensive healthcare services within a primary care clinic, working closely with physicians and the medical director to assess, diagnose, and treat patients. This role emphasizes patient-centered care, preventive health, and effective management of medical conditions to promote overall wellness.

Responsibilities

• Conduct patient assessments to evaluate health status and gather medical histories
• Develop and implement treatment plans based on clinical diagnosis
• Manage and prescribe medications as appropriate for patient care
• Provide patient education on health maintenance and disease prevention
• Coordinate care among healthcare providers to ensure seamless patient management
• Monitor patient progress and adjust care plans accordingly
• Maintain accurate and thorough clinical documentation in Electronic Health Records (EHR)
• Promote preventative care strategies to improve patient outcomes

Preferred Qualifications

• Minimum of 2 years' experience as a Nurse Practitioner
• Master's Degree in Nursing or related field
• Active Nurse Practitioner (NP) license
• Proficiency in patient assessment, clinical diagnosis, and medication management
• Experience with Electronic Health Records (EHR) systems
• Strong patient education and communication skills
• Critical thinking with the ability to collaborate effectively within a healthcare team
