Clinical Pharmacist

Your OpportunityWe are seeking a Clinical Pharmacist to be a key contributor across enterprise clinical programs, supporting provider and patient engagement, medication optimization, clinical decision support programs, and drug strategy initiatives. This role partners closely with medical leadership, care teams, and operational stakeholders to improve quality, outcomes, and total cost of care in a value-based environment.The ideal candidate combines strong clinical expertise with the ability to drive program execution, collaborate cross-functionally, and translate data-driven insights into real-world clinical impact.Clinical Programs & Patient EngagementOperate within pharmacist-led clinical programs focused on chronic disease, quality improvement, and medication optimization.Activate on provided lists of eligible patients by outreaching, enrolling, and engaging patients. Maintain engagement through education, outreach, and follow-up.Conduct Medication Reconciliation, Medication Therapy Management (MTM), and Comprehensive Medication Reviews (CMR).Provide telephonic patient education on medication use, adherence, self-management, and escalation of care.Provide specific actionable medication recommendations and collaborate with providers, using technology and direct communication, to support practice adoption and medication management.Support other clinical programs to provide periodic consultation on pharmacy related issues affecting patientsDrive medication adherence to address quality gapsCollaborating with the IDT—which includes primary care physicians, health coaches, and nursing staff—to develop patient-specific clinical action plans, reconcile medications, and make necessary adjustments to medications.Clinical Decision SupportAccelerate adoption of clinical insights by providers by driving uptake of actionable drug-related recommendations.Provide clinical feedback to enhance decision support logic, usability, and impact.Cross-Functional CollaborationCollaborate with Regional Medical Officers, Market Medical Directors, Practice Transformation Managers, care teams, and practices.Support consistent clinical standards, workflows, and messaging across markets.Drug Programs StrategySupport ideation and development of initiatives related to specialty medications, high-cost drugs, and pharmacy optimization programs.Partner with pharmacy and medical leadership on drug strategy aligned with value-based care goals.Compliance, Documentation & QualityDocument all clinical activity accurately and in accordance with state and federal regulations and HIPAA requirements, using provided platforms and technologiesServe as a pharmacy information resource for providers and internal clinical teams.Remain current on clinical guidelines, quality measures, and pharmacy best practices.What Will Make You Successful HereDoctor of Pharmacy (PharmD).Active pharmacist license in good standing (state-specific: CA,WA, KS, OH, NY, NJ, FL, MD, NH, MS, LA).UMPJE (Preferred)Experience in Medication Therapy Management and pharmacist-led clinical programs.Strong understanding of evidence-based guidelines and quality measures.Experience or interest in value-based care, population health, or ACO environments.Skills & AttributesStrong clinical judgment with ability to make independent recommendations.Comfortable working across clinical programs, data-driven tools, and operational teams.Comfortable working with partial or evolving clinical informationAbility to drive results, not just provide clinical input.Excellent communication skills with providers, patients, and internal stakeholders.Organized, adaptable, and effective in fast-paced, evolving environments.High degree of professionalism, integrity, and accountability.Perks/BenefitsCompetitive base compensationAnnual bonus potentialHealth benefits effective on start dateHealth & Wellness Program; up to $300 per quarter for your overall well-being available on start date401K plan effective on the first of the month after your start date; 100% of up to 4% of your annual salary5 sick days and unlimited (or generous) paid "Vytal Time" after your first 90 daysCompany paid STD/LTDTechnology setupAbility to help build a market leader in value-based healthcare at a rapidly growing organizationPlease note at no time during our screening, interview, or selection process do we ask for additional personal information (beyond your resume) or account/financial information.
