Responsibilities: Develop and execute community outreach strategies to identify and recruit eligible patients Build and maintain relationships with community organizations, healthcare providers, and local partners Conduct outreach activities at community events, health fairs, and partner locations Screen and enroll prospective patients into programs, ensuring eligibility requirements are met Maintain accurate records of outreach activities, patient contacts, and recruitment metrics Serve as a trusted resource and point of contact for patients navigating available services Collaborate with internal teams to align outreach efforts with organizational goals Requirements: Experience in community outreach, patient recruitment, or a related healthcare/social services role Strong interpersonal and communication skills with the ability to connect with diverse populations Knowledge of local Glendale/Phoenix Metro community resources and healthcare landscape is a plus Ability to work independently, manage time effectively, and meet recruitment goals Proficiency in maintaining accurate documentation and data entry Bilingual (English/Spanish) preferred but not required Valid driver's license and reliable transportation for community-based activities About Us: We are a community-focused organization based in Glendale, AZ, dedicated to improving lives through innovative programs and patient-centered services.
