Primary Care Physician - Internal Medicine / Family Medicine Location: Anaheim, CA Job Type: Full-Time Schedule: Monday-Friday | Day Shift Setting: Outpatient Position Summary We are seeking a Board-Certified Primary Care Physician in Internal Medicine or Family Medicine to provide comprehensive, patient-centered care to adult and geriatric populations in an outpatient setting. The physician will work collaboratively with an interdisciplinary healthcare team to assess patients, develop individualized treatment plans, manage chronic conditions, and ensure high-quality ongoing care. This is an excellent opportunity for a physician passionate about caring for frail and elderly patients . Key Responsibilities Provide comprehensive primary care, including evaluation, diagnosis, treatment, and chronic disease management. Perform physical examinations and review medical histories. Order and interpret appropriate diagnostic tests and develop individualized treatment plans. Manage medications, preventive care, and health-promotion needs. Collaborate with an interdisciplinary team to coordinate and monitor patient care. Educate patients and families regarding health maintenance, medical conditions, and treatment plans. Coordinate specialist referrals and follow-up care as needed. Supervise and collaborate with Nurse Practitioners and Physician Assistants. Maintain accurate and timely medical documentation. Participate in quality improvement initiatives and support compliance with healthcare regulations and organizational policies. Qualifications MD or DO from an accredited medical school. Active California medical license required. Current DEA registration required. Board Certified in Internal Medicine or Family Medicine required. Minimum 1 year of experience working with frail or elderly populations. Geriatric care experience or additional qualifications in geriatrics required.
