Weekend Admission Nurse

Part-time Registered Nurse Case Manager Anticipated Territory: Santa Clara County: San Jose* Schedule: Saturday/Sunday What you’ll do: Conduct Start of Care (SOC) Admissions - OASIS Conduct follow‑ups and evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including medication reconciliation, wound care, treatments, and procedures Monitor clients’ conditions and report changes to the Clinical team Collaborate with physicians and agency staff to deliver the best care possible Accurately document observations, interventions, and evaluations related to patient care management and services provided Use concierge resources that assist with time‑consuming administrative tasks Create a flexible schedule and determine your own work hours Target patient visits within a 5–15 mile radius of the primary territory Enjoy autonomy with no clocking in/out Field additional coverage occasionally outside the primary territory as needed for staffing or patient census Qualifications Current and unrestricted California Registered Nurse license Current BLS/CPR certification Valid California driver’s license Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired High level of clinical knowledge in home health care is highly preferred Experience with high‑acuity patients is a plus Experience working with an interdisciplinary team is highly desired Case Management fundamentals is highly desired Self‑directed, empathetic, and a critical thinker Outstanding interpersonal skills and leadership ability Organized and detail‑oriented Benefits Mileage/Vehicle reimbursement Company‑issued iPad and helpful software/communication tools Competitive compensation with incentive bonus program Outstanding benefits including 401(k) with matching, generous PTO, medical, dental, and life insurance Low turnover with high morale and support for work/life balance * You will be assigned a primary location, but may occasionally be asked to provide coverage outside that area as needed for staffing, leaves of absence, higher patient census, or other client care needs.
