Weekend Admission Nurse (RN) - Home Health

All opportunities at 21HHS require being in the field visiting patients in their homes. This position is a Part‑time Registered Nurse Case Manager, anticipated territory: Sacramento County. Schedule: Every Saturday and Sunday full days, with potential Saturday/Sunday plus 3 weekdays. What you’ll do: Conduct Start of Care (SOC) Admissions – OASIS Conduct follow‑ups, evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions, reporting changes to Clinical team Collaborate with physicians and agency staff to deliver the best care possible Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols. Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
