Job Summary:

Essential Functions: 

•Set up appointments for CASAC if necessary

•Record assessment information into access database 

•Schedule CASAC reviews

•Obtain all attendance sheets for open D/A cases

•Monitor clients progress in treatment

•Schedule reviews with CASAC as determined

•Maintain files on all clients with current paperwork, releases, attendance and discharges 

•Complete monthly statistic logs 

•Complete semi – annual RBA report 

•Work with employment and under care to ensure clients remain in compliance 

•Send out appointment letters and other communication to clients

•Completing employment assessments and restoration to self-sufficiency plans

•Assisting individuals with locating the appropriate treatment based on CASAC recommendations and the district’s final determination.

•Work with drug court, employment and under care to ensure clients remain in compliance 

•Participate in staff meetings, training sessions, and professional development activities as required.

•Other duties as assigned. 

Requirements:

Qualifications:

•Bachelor's degree in social work, psychology, counseling, or a related field (Master's degree preferred).

•Proven experience in community service coordination or related social services work.

•Strong understanding of substance abuse disorders, mental health issues, and trauma-informed care principles.

•Excellent communication, interpersonal, and organizational skills.

•Ability to work independently and as part of a team in a fast-paced environment.

•Current and valid New York State driver’s license
