Case Manager

UnitedHealthcare is seeking a Case Manager for LTSS in Colorado. You will perform intake, assessments, care planning, and ongoing case management, documenting activities in the prescribed system and coordinating with supervisors. Travel to member homes and facilities is required, with face-to-face visits and reassessments as needed.
