Substance Use Disorder Counselor

Health Care Resource Centers, part of HCR Home Care, is seeking a Substance Use Disorder Counselor to deliver person-centered counseling and case management in home and community settings. In this role, you will complete biopsychosocial assessments, develop individualized treatment plans, provide individual and group counseling, and coordinate care with medical and behavioral health providers. You will use evidence-based practices, maintain timely clinical documentation, educate families, and support relapse prevention while upholding HCR's values of respect, integrity, safety, and ethical care. Responsibilities Conduct comprehensive substance use and mental health assessments Develop and update individualized treatment and recovery plans Provide individual, group, and family counseling using evidence-based modalities Deliver services in patient homes and community settings as appropriate Coordinate care with physicians, nurses, social workers, and community resources Monitor progress, adjust interventions, and support relapse prevention Maintain accurate, timely, and compliant clinical documentation Educate patients and families on substance use, harm reduction, and recovery resources Participate in interdisciplinary team meetings and case conferences Adhere to all regulatory, ethical, and safety standards in home care environments Required Skills Substance use assessment and screening (e.g., ASI, SBIRT) Individual and group counseling Motivational Interviewing (MI) Cognitive Behavioral Therapy (CBT) techniques Treatment and recovery planning Co-occurring disorder identification and referral Clinical documentation and EMR proficiency Crisis intervention and de-escalation Knowledge of addiction medications and MAT coordination Care coordination and community resource navigation #J-18808-Ljbffr
