Key Responsibilities\n

• Conduct thorough psychosocial assessments using established methodologies to identify clients' mental health needs.

• Develop, implement, and adjust individualized treatment plans in collaboration with clients, their families, and healthcare teams.

• Provide counseling services to help clients manage mental health challenges, including depression, anxiety, and life transitions.
\n
• Facilitate case management efforts to link clients to necessary community resources, including in-home services, support groups, and other healthcare modalities.
\n
• Monitor client progress through regular follow-up sessions, documenting changes in mental health status and updating treatment plans accordingly.
\n
• Educate clients and their families on mental health topics and coping strategies to enhance their understanding and management of their conditions.
\n
• Participate in collaborative care meetings with interdisciplinary teams, including psychiatrists, nurses, and primary care providers, to ensure holistic approaches to client care.

• Stay current with mental health best practices, research findings, and legislative changes to uphold ethical standards in practice.

In this role, you will primarily work with diverse populations facing a variety of mental health challenges. Qualifications\n

• Master's degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW).

• A minimum of two years of experience in a mental health counseling role, preferably in a community health or clinical setting.

• Demonstrated proficiency in assessment, diagnosis, and treatment of mental health disorders.

Compensation and Benefits\n
