At Care New England, we’re looking for dedicated PTs to join our Home Health team and help patients regain strength, mobility, and independence where they feel most comfortable. What You’ll Do As a Home Health Physical Therapist, you’ll manage a caseload of patients from admission through discharge, providing comprehensive evaluations, developing individualized treatment plans, and delivering therapeutic interventions. You’ll collaborate with a supportive interdisciplinary team to achieve the best possible outcomes for each patient.
