Overview At The New York Foundling, a New York‑based nonprofit founded in 1869, we support children and families through and beyond foster care, conflict, poverty, developmental disabilities, and access to health services. Our interrelated programs provide a whole‑person, whole‑family, and whole‑life approach to build resilience and wellbeing. The Foster Care Case Planner works toward permanency for children through reunification, adoption, or independent living. The role involves implementing evidence‑based models, conducting birth home and foster home visits, providing community service referrals, accompanying families to treatment and school services, attending court hearings, and preparing reports. Responsibilities Engage birth parents, foster parents, and foster youth using motivational interviewing, assess safety and risk factors. Implement evidence‑based and evidence‑informed models; participate in family conferences and supervise family visits. Serve as primary liaison for families in medical, drug treatment, screening, school, and housing settings. Collaborate closely with socio‑therapist. Advise, counsel, advocate, and case plan with parents and discharge resources and children in care. Provide family‑planning counseling to youth 12 years and older, documenting discussions. Develop safety plans and apply crisis‑intervention skills in a trauma‑informed approach. Document progress notes, reports, FASPs, and plan amendments via Connections within required time frames. Maintain access to Connections and other database systems, ensuring compliance with New York state, city, and Foundling guidelines. Attend team meetings, case consultations, and individual and group coaching. Collect documentation and compile applications for special rate or housing services. Maintain a caseload of about 10 active and 2 suspended cases. Attend all court proceedings on time, prepare reports, and testify when required. Conduct home visits to foster and birth homes and coordinate family and sibling visitation/meetings. Advance outreach and advocacy services, engaging absent or hard‑to‑reach family members and providing referrals. Perform any other duties as assigned. Qualifications BA/MA in Psychology, Sociology, or related field; BSW or MSW with 1–2 years child welfare experience preferred. Knowledge of community resources and ability to negotiate systems for multi‑problem families. Experience with drug treatment populations preferred. Bi‑lingual Spanish/English preferred. Typing skills and computer literacy required. Excellent interpersonal skills. Education Required Bachelor’s Degree from a U.S. accredited institution or credential evaluation for a non‑U.S. institution.
