Nurse Practitioner 2

The UCSF Neurology Program seeks a full time Advanced Practice Provider (APP) to help support our program. The UCSF team consists of highly experienced physicians, surgeons, nurses, social workers, researchers and others. Our Neurology program is nationally known for cutting edge Neurology support in both the outpatient and inpatient setting and offers a variety of support systems (long-term and short-term).

The APP will care for patients on the inpatient Neurology services at UCSF, Division of Neurology. Specific duties will include rounding with the inpatient team, answering pages from nurses, writing orders, writing notes (admit H and P, discharge summaries, significant event), evaluation and management of patients and working with care team members for discharges. Will also include evaluation and management for recently discharged patients as they are establishing outpatient care. This job will provide opportunity to gain skills in evaluating neurological patients, perform neurological examinations and manage neurology conditions as part of a team-based care model.

Participate in managing hospitalized patients in the ICU and floor settings, coordinating with other team members. Hospital Setting:

1. The APP rounds and is authorized to:

   1. Examine patients

   2. Record subjective and objective findings in the patient chart and report findings to the physician.

   3. Check laboratory results, vital signs, intake/ output, pain scale, and report the results to the physician.

   4. Order medications on the approved medication list.

   5. Provide wound management, examine for signs of infection, order and change dressings, order or remove sutures or staples.

VI. Order speech therapy, respiratory therapy, physical therapy, psychological therapy, and occupational therapy, as appropriate.

VII.Refer patients to social work services, nutrition service and home health care.

VIII.Refer patients and call on consultation from medical specialties.

IX Order nutritional supplementation and enteric nutrition.

X.Order laboratory tests and radiologic studies.

XI.Write transfer orders to appropriate levels of care.

XII Removal and placement of chest tubes, internal jugular lines, central lines, intra- operatively placed drains, surgical dressings, and Foley catheters.

XIII Provide patient teaching, counseling, and discharge planning.

XIV. Obtain arterial blood gas and blood cultures.

XIV. Document in database and hospital Electronic Health Record system as directed by UCSF Health standard policies and procedures.

XV. The APP while concomitantly notifying the attending physician will initiate management of complications.

Care coordination/Clinical Review/Administrative Duties:

1. Clinical documentation and patient care follow up

   1. Patient advice requests that are escalated from triage RNs, administrative staff or other care providers

   2. In basket management - results review, expiring orders, telephone encounters, patient advice requests, cc'd charts.

   3. Patient emails: Answer patient emails escalated to provider for further evaluation.

   4. Participate in neurology-related CME activities.

   5. Support development of unit-based procedures, clinical practice standards, provide in-house education through informal and formal educational sessions, and support the teaching mission through APP student placements in the clinic when possible.

   6. Other

The Advanced Practice Provider (NP/PA/CRNA/CNM) is authorized to order, adjust, and furnish medications as appropriate for the care of the patient and according to the formulary listed in Standardized Procedures

• To assist in formulating and implementing protocols to improve efficiency in the clinical specialty/service/pathways.

• Engage in a timely and efficient discharge of patients on the inpatient service for which you are responsible.

• Assist in coordinating patient trafers withing and outside UCSF Health according to policy

• Be familiar with outcome metrics, guidelines, and expectations for providers in clinical specialty

• Performs other duties as assigned.

• This positions requires flexibility for the provider to work at all UCSF Health licensed facilitities

• Support outpatient team as needed/indicated to ensure quality care of patients.

A Nurse Practitioner is an advanced practice registered nurse who possesses the advanced education, knowledge, clinical skills and experience to provide expert advanced nursing care in their area of specialization. They will be guided and follow the policies and standardized procedures, rules and regulations, requirement of supervision under CA statute, and bylaws of the UCSF Medical Center, some Nursing, Medical Staff and unique policies related to the provision of patient services. In California, nurse practitioners can practice, bill, and provide independent services under a standardized procedure in collaboration with a supervising physician.

Required:

• One (1) year of employment as a Nurse Practitioner.
• Flexibility to work in varying schedules as outlined (days/weekends/holidays rotations)

Preferred Qualifications

• One (1 )year of experience as a Nurse Practitioner in neurology
• One (1) to Two (2) years of experience as UCSF nurse or APP in acute care /critical care setting

License/Certifications:

• California driver's license (CDL) if required for position.
• Current national certification from an accepted certifying agency and provide by first day of work.
  • Accepted Certifications for this Adult position. Adult inpatient ( ACNP-BC, AGACNP-BC, ACNPC-AG, ACNPC)
• Current and active RN and NP and Furnishing license in California.
• Complete a BRN recognized graduate master's degree program as a nurse practitioner.
• Drug Enforcement Administration (DEA) Registration with Schedules 2, 2N, 3, 3N, 4, and 5 registered to a California address
• Submit enrollment with CMS Medicare and Medicaid through credentialing by first clinical day. CCS enrollment as appropriate for pediatric environments.
• Current AHA BLS.
• Current AHA ACLS.
• State regulation requires that you have a supervising physician to practice.
• Incumbents with less than 6 months of NP experience will be hired as a Nurse Practitioner 1.

UCSF Medical Center and Campus requires certain healthcare providers to be credentialed. In most cases this is done through its Office of Medical Affairs & Governance (OMAG) prior to the first working day. Credentialing and approval of the Standardized Procedures takes between 60-90 days but can vary.
