A well-established skilled nursing facility (SNF) in Roselle Park, New Jersey is hiring a full-time MDS Coordinator (RN) . This is a dedicated MDS Coordinator role, not a floor position with MDS tacked on. The right MDS Coordinator here is a Registered Nurse (RN) who already knows the Minimum Data Set (MDS) process cold and wants a stable building where they can own it.
Prior nursing home experience is required. We are looking for an MDS Coordinator who has actually completed assessments in a skilled nursing / long-term care (LTC) setting and is comfortable with PDPM (Patient Driven Payment Model) reimbursement. What You'll DoComplete and submit accurate, timely MDS assessments under the Resident Assessment Instrument (RAI) process, including scheduling and tracking of all assessment types
Coordinate with nursing, therapy, dietary, and social services to capture the full clinical picture for each resident
Oversee care plan development and ensure documentation supports coding
Monitor PDPM reimbursement accuracy and Quality Measures, and flag issues before they become problems
Support survey readiness through clean, defensible MDS documentation
Active Registered Nurse (RN) license in New Jersey
Hands-on MDS Coordinator experience in a skilled nursing facility (SNF) / long-term care (LTC) setting is required
