Harlem New York - Day Service Supervisor Harlem, New York, NY 10027, USA Job Description Provide direct supervision to Habilitation Specialists and Direct Support Professionals in Community Prevocational and Day Habilitation Services for individuals with Intellectual and Developmental Disabilities. Responsibilities Supervise up to 30 individuals with I/DD, ensuring Person-Centered Planning and service delivery. Coordinate scheduling of group and individual activities and services. Complete required documentation (Life Plans, Monthly Case Notes, Person Centered Plans, Prevocational Assessments, DDP1’s, Incident Reports, etc.) to funding source and oversight agency standards. Ensure monthly attendance goals for individuals on caseload and prepare attendance reports. Conduct assessments, develop Staff Action Plans, Employment Plans, and goals based on individual progress. Provide on-site guidance, leadership and administrative oversight; including petty cash management, Metro Card distribution, transportation reimbursements, and monthly fire drills. Visit hub, volunteer and internship sites to supervise activities, evaluate progress, and monitor service quality. Ensure Rehabilitation Technicians provide daily services, record time & attendance accurately. Adapt communication to meet individuals’ needs and maintain professionalism, exercising good judgment in crisis situations. Recruit new participants, liaison with educators, transition coordinators and transportation company. Participate in annual Life Plan meetings, provide oversight, guidance and discipline for technicians. Assist with hiring and disciplinary actions; maintain audit-ready charts and work with QA and CHOICES & IRMA. Manage overall site operations and safety. Qualifications Bachelor’s degree in a related field preferred or equivalent combination of experience. Minimum one year of experience in mental health, vocational rehabilitation or case management. Strong interpersonal, communication, organizational and time-management skills. Ability to handle multiple tasks simultaneously, prioritize demands, work independently with minimal supervision. Flexible, open to new ideas, solution-oriented, and avoid making assumptions about individuals’ abilities. Ability to handle confidential information with discretion and judgment. Working Conditions Frequent travel and field work within boroughs; attendance at evening meetings and off-site locations; onsite presence required.
