We’re looking for a Resident Services Coordinator to join our Bonita Villa Senior Living team!

We are seeking a compassionate, organized, and detail-oriented Resident Services Coordinator to support our Resident Services team in providing exceptional care and service to our residents. This position plays a key role in coordinating resident care, maintaining accurate health records, overseeing medication management processes, and supporting the daily operations of the Resident Services department.

Position Summary

The Resident Services Coordinator assists the Resident Services Director with monitoring resident health and wellness while ensuring accurate documentation, regulatory compliance, and effective communication among residents, families, healthcare providers, and team members.

Primary responsibilities include:

• Maintaining resident health records and documentation
• Coordinating Personal Care Assistant schedules and daily assignments
• Monitoring self-medication programs and maintaining medication records
• Creating and maintaining resident emergency packets
• Organizing and updating resident charts
• Supporting communication between residents, families, physicians, pharmacies, and community staff

Essential Duties and ResponsibilitiesResident Care & Health Services

• Monitor residents' health status and report significant changes to the Resident Services Director or Executive Director as appropriate.
• Notify physicians, pharmacists, and families regarding changes in resident condition, medication reactions, or significant incidents.
• Maintain accurate, timely, and confidential resident records in accordance with community policies and applicable regulations.
• Update medical records, resident charts, and emergency information.
• Monitor and audit medication administration records for accuracy and compliance.
• Coordinate medication orders with pharmacies and ensure medications are properly stored.
• Monitor self-medication programs and maintain required documentation.
• Follow up on weekly bath schedules, vital signs, weights, blood pressure, and other health monitoring as assigned.
• Review incident reports and assist in implementing interventions to reduce future risks.
• Ensure resident files are complete and contain all required signatures.

Scheduling & Administrative Support

• Schedule Personal Care Assistants and prepare daily assignments.
• Organize and maintain resident charts and emergency packets.
• Maintain inventory of medical and office supplies.
• Ensure medical equipment is maintained in safe working condition.
• Serve as a communication liaison during assigned shifts between staff, physicians, residents, and families.
• Assist with departmental administrative tasks as assigned.

Team Collaboration

• Support the community's mission, vision, and values through positive leadership and communication.
• Foster professional relationships with residents, families, physicians, pharmacies, vendors, and fellow team members.
• Demonstrate flexibility by assisting other departments and covering resident services shifts when needed.
• Attend required meetings and in-service training.
• Report concerns, incidents, and operational issues promptly to supervisors.
• Perform additional duties as assigned.

Qualifications

Required:

• High school diploma or equivalent.
• Certified Nursing Assistant (CNA) certification.
• Current First Aid and CPR certification.
• Previous experience in an assisted living community, memory care, or skilled nursing facility.
• Valid driver's license, reliable transportation, and automobile insurance as required by law.
• Ability to read, write, speak, and understand English fluently.
• Strong organizational, communication, and documentation skills.
• Proficiency with electronic health records and basic computer applications is preferred.

Physical Requirements

The employee must be able to:

• Stand, walk, bend, stoop, and reach frequently throughout the workday.
• Lift up to 20 pounds and push up to 40 pounds safely, using appropriate lifting techniques and equipment.
• Maintain mental alertness, attention to detail, and sound judgment.
• Effectively communicate with residents, families, staff, and healthcare professionals.
• Possess close vision, depth perception, and the ability to adjust focus.
• Respond quickly and appropriately during emergency situations.
• Move throughout the community to assist residents and staff as needed.

Working Conditions

• Must be available to work scheduled shifts, including evenings, weekends, holidays, and overtime as needed.
• Work is performed in a residential healthcare environment with moderate to occasionally high noise levels.
