Part Time RN Case Manager- In-Home Infusions/Injections Benefits: Paid Mileage Employee Appreciation Programs Weekly Pay Flexible schedule Opportunity for advancement Paid time off Training & development BrightStar Care is looking for a dedicated and compassionate Part Time RN Case Manager to provide in-home infusion and injection services to patients in the Minneapolis and surrounding areas. This flexible position requires availability 3 days a week between 8:00 AM – 5:00 PM, with travel within a 30-mile radius . The ideal candidate would be willing to case manage 4‑5 clients. Infusion and injection visits are scheduled based on treatment plans and your availability. Compensation is $70 for 1-hour visits, $50 for the first two hours of longer visits, and $45 per hour thereafter. Mileage pay may be offered on a case-by-case basis. Job Responsibilities: Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards. Collaborate with multidisciplinary team members, partnering pharmacies and providers to optimize patient care and outcomes. Educate patients and their families on health maintenance and disease prevention. Ensure accurate documentation and compliance with all regulatory guidelines. Requirements: Active RN license in the state of Minnesota. Previous experience in a home health or acute care setting is advantageous. Exceptional interpersonal and communication skills.
