This full-time role is offered in a remote setting, allowing you the flexibility to work from home while connecting with colleagues and clients virtually. Regular business hours will typically be observed, but you may be required to accommodate occasional evening and weekend hours for special programs or community events. Your work will primarily focus on providing therapeutic services in our Hospice Center, engaging with clients who are navigating complex emotional challenges.

Key Responsibilities\n

• Conduct comprehensive assessments to determine clients' social, emotional, and psychological needs, ensuring that each assessment is thorough and reflective of the individual's circumstances.

• Deliver individualized and group counseling sessions employing evidence-based therapeutic approaches, helping clients to set and achieve their personal goals.

• Develop and implement tailored service plans that outline clear objectives for client care and monitor progress towards these goals.

• Act as a strong advocate for clients—from securing necessary resources to addressing and fighting for their rights—focusing on social justice issues that impact their lives.

• Participate actively in program development and evaluation, including grant writing and reporting to ensure our services continue to meet community needs.

• Engage with community resources and organizations to enhance service offerings, foster collaborative efforts, and address systemic issues impacting our clients.

• Contribute to team meetings and training sessions, sharing knowledge and strategies while actively participating in community outreach activities to raise awareness about available resources.

Patient Population / Client Focus\n

Your primary focus will be on individuals and families experiencing a variety of mental health challenges within the hospice setting. You will provide support and services to clients facing end-of-life issues, grief, and trauma, requiring a compassionate understanding of their emotional and psychological states. Your role will be critical in ensuring that clients receive the support and resources they need during this sensitive period in their lives.

Compensation and Benefits\n
