Key Responsibilities\n

• Conduct thorough assessments of clients experiencing major mental illnesses or severe emotional disorders, both in office and community settings.

• Work collaboratively with a multidisciplinary team to establish and pursue common goals, reinforcing a shared commitment to teamwork and cross-functional communication.

• Develop and implement individualized integrated care plans, ensuring clients have access to necessary linked services and facilitating communication among clients, families, and providers.

• Provide a variety of prevention, treatment, and recovery services tailored to client needs, including crisis intervention, individual therapy, group therapy, and family therapy.

• Monitor client progress through regular follow-up meetings, ensuring services align with presenting problems and client requests.

• Complete required documentation and reporting in a timely manner to support continuity of care and data submission.

• Perform brief, evidence-based screenings and assessments as necessary, adapting approaches to meet clients’ developmental and cultural needs.

• Participate in on-call duties as assigned, ensuring continued support for clients during and after program hours.

• Engage in quality improvement initiatives within the organization to enhance service delivery and address evolving client needs.

Qualifications\n

• Must hold a Bachelor’s Degree in Social Work or a related field.

Compensation and Benefits\n
