Work Environment

As a Nurse Practitioner at SLI Rehab & Physical Therapy, you will work onsite in a school setting, which provides a unique opportunity to connect with students and their families. Our facility is equipped with modern medical tools and technology that allow for efficient patient care and health monitoring. You will collaborate with other healthcare providers and educators to create a supportive environment conducive to learning and health enhancement. Your work schedule typically spans 9:00 a.m. to 6:00 p.m., with potential evening clinics until 8:00 p.m., ensuring flexibility to meet community needs, including participation in weekend health fairs or outreach events.

Key Responsibilities

• Conduct comprehensive health assessments, including physical examinations and diagnostics, for individuals and families in the community.
• Diagnose and manage a variety of acute and chronic medical conditions, focusing on preventive care and health promotion strategies.
• Design and implement community health education programs in collaboration with local organizations, fostering increased awareness and proactive health behaviors among community members.
• Provide culturally sensitive and patient-centered care while advocating for the rights and access to healthcare services for all individuals, particularly marginalized groups.
• Coordinate referrals and ensure follow-up care for patients to maintain continuity in their treatment plans.
• Maintain accurate and detailed medical records in compliance with healthcare regulations, ensuring relevant patient information is securely stored and easily accessible.
• Engage in community events and health fairs, providing valuable health screenings and information to promote community wellness.
• Stay informed about emerging trends in community health and evidence-based practices to ensure that services remain current and relevant.

Patient Population / Client Focus

Your efforts will primarily focus on families and individuals residing in the Oswego community, particularly those who may have limited access to healthcare services. You will encounter a diverse patient population, including children in the school system, adults, and elderly patients, each with unique health challenges and educational needs. Qualifications

• Current certification as a Nurse Practitioner.
• Valid state nursing license to practice in New York.
• Bachelor's degree in nursing from an accredited institution.
• Master's degree in nursing from an accredited program.

• A minimum of 3-5 years of clinical experience, particularly in community health or public health settings.

Compensation and Benefits

This position offers a competitive salary range of USD 180,000 to 200,000 per year, based on experience and qualifications.
