Position Overview

Gotham Enterprises Ltd is actively seeking a dedicated and skilled Nurse Practitioner (NP) to join our Center for Advanced Cardiac Care (CACC) in Oswego, New York. This full-time, onsite position offers professionals the opportunity to work in a collaborative environment focusing on outpatient care within the cardiology division. You will be under the management of our Chief Nurse Practitioner and receive guidance from the CACC Medical Director, contributing directly to the health and well-being of patients with complex cardiac conditions.

This position is based onsite within our home care setting, allowing you to engage directly with patients in a dynamic outpatient environment. Key Responsibilities

In your role as Nurse Practitioner, you will have a diverse range of responsibilities, including but not limited to:

• Conducting comprehensive patient assessments, including obtaining detailed health histories and performing targeted physical examinations.
• Ordering and interpreting diagnostic tests to formulate accurate diagnoses and develop effective management plans for both acute and chronic cardiac conditions.
• Facilitating patient care autonomously while also collaborating with physicians and NP colleagues to enhance treatment protocols.
• Reviewing internal and external medical records to inform treatment decisions and improve continuity of care.
• Creating and managing goal-directed care plans that focus on health maintenance, patient education, and coordinated nursing interventions in line with established clinical protocols.
• Following up with healthcare providers to assess treatment plan efficacy and make necessary adjustments where needed.
• Providing counseling and educational support to patients and their families regarding disease management and outpatient care strategies.
• Coordinating with inpatient teams for safe transitions through hospital admissions and discharges.
• Triage and manage daily patient calls effectively, ensuring timely communication and support with referring providers.
• Documenting all patient encounters meticulously within the electronic medical records (EMR) system to ensure clear communication across care teams.
• Participating in clinical research protocols and contributing to ongoing learning and improvement efforts within the practice.
• Maintaining a safe and therapeutic environment for patients and staff.

Patient Population / Client Focus

You will primarily serve adult patients experiencing complex cardiac issues, including those with advanced heart failure, pulmonary arterial hypertension, and individuals requiring post-heart transplant care. Your expertise will be vital for patients undergoing advanced treatment therapies such as intravenous and inhaled medications or mechanical circulatory support, helping them navigate their healthcare journeys effectively.

Qualifications

The ideal candidate will possess the following qualifications:

• A Master's degree in Nursing with specialized training as a Nurse Practitioner focusing on adult care.
• A minimum of two years of experience working in a clinical or hospital setting, preferably with a cardiology background.
• Current New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Board certification as a Nurse Practitioner, along with BCLS and ACLS certifications.
• Possession of a valid DEA and NPI certification.
• Proficiency in electronic medical record systems and Microsoft Office Suite (Word, PowerPoint, Excel) to facilitate documentation and presentations.

Preferred Experience

Experience with complex patient populations, specifically those dealing with advanced heart conditions, is highly desired. Familiarity with coordinating care around advanced therapies will be beneficial in enhancing patient treatment outcomes and supporting families effectively.

Compensation and Benefits
