We are expanding our Care Partner team and looking for adaptable Certified Medical Assistants to join our Float Pool. In this role, you will support patients across multiple medical practices rather than being assigned to a single practice. You’ll work with different teams, adapt to changing workflows, and help ensure continuity of care across our network.

Key Responsibilities: - Regularly engage with patients and track their adherence to their care plans. - Closely monitor patients' device readings and dig into any out-of-bounds values. - Maintain meticulous records of patient interactions and program details. - Stay up to date on the latest regulations and compliance standards for CCM/RPM services. - Ensure all patient enrollments adhere to legal and ethical standards. - Monitor and track care performance, striving to meet or exceed goals. - Support patients across multiple medical practices, adapting to different workflows, teams, and patient populations. - Transition seamlessly between practices based on operational needs while maintaining consistent care quality. Qualifications: - National Certification in Medical Assisting. - Residence in the US. - Strong written and verbal communication skills. - Empathy and the ability to connect with patients and their families. - Basic knowledge of chronic health conditions and their management. - Highly adaptable and comfortable working across different medical practices and teams. - Strong interpersonal and communication skills, with the ability to communicate clearly and naturally with colleagues. - Reliable, flexible, and comfortable working in a float pool environment. - Comfortable using technology for documentation and communication. - Strong attention to detail and organizational skills. - Ability to work independently and as part of a team.
