Care Manager

As a Care Manager, you will play a key role in ensuring the safety, health and well-being of clients in the home environment. You will report directly to the Lead Care Manager. Pay: $30-34/hr Key Responsibilities Collaborate with the Care Management team and staffing personnel to create effective care and staffing strategies. Conduct health and home assessments for new and prospective clients. Build relationships with prospects over the phone and schedule assessment visits. Provide orientation and ongoing training for caregiver team members, including specialized instruction when needed. Assess client needs and create individualized care plans that uphold our core values. Exercise strong clinical judgment and maintain a high standard of documentation. Qualifications Current, unencumbered Missouri RN, LPN license, MSW, or BSW with related experience CCM or CMC preferred but not required 2 plus years of experience working with geriatric patients/clients Valid driver\'s license with a good driving record Ability to work independently while providing excellent professional judgment Strong assessment, documentation, and careplanning skills Proficiency with Microsoft Office Why you\'ll love working for us Manageable Caseload - Avoid burnout and give each client the attention they deserve Make a Real Difference - Improve the client\'s dignity, independence, and quality of life.
