Nurse Practitioner OpportunityFairview is seeking a Nurse Practitioner to join our primary care practice at M Health Fairview Clinic – Bass Lake. Located in the growing Maple Grove/Plymouth communities, M Health Fairview Clinic – Bass Lake provides comprehensive, patient-centered primary care for patients of all ages. The clinic offers a collaborative and supportive environment focused on whole-person care, with services including family medicine, pediatrics, onsite lab, and imaging for added convenience.As part of continued growth and expansion in the northwest metro, the current Bass Lake clinic will transition to a new, larger clinic location with expanded services and increased capacity, anticipated to open in January 2027. In preparation for this exciting growth, we are actively recruiting additional physicians to support the expanding patient population and future care model.Practice Details:9 provider primary carePatient encounters average 18-20 patients per day.1 FTE = 34 patient contact hours per week.Openings: 1.0 FTE (Full Time) or less - Flexible schedule.As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.Fully integrated Electronic Medical Record (EPIC).Responsibilities:Assess health status for specific patient populations using advanced practice knowledge and skills.Obtains relevant health and medical history.Performs a physical examination based on age and medical history.Performs/orders preventative and diagnostic procedures based on the patient's age and history.Identifies health and medical risk factors.Identifies acute and chronic health problems.Appropriately orders and accurately interprets diagnostic results.Develops and implements an individualized plan of care.Establishes practice agreements with collaborating physician.Collaborates with health care team members in developing realistic care expectations.Determines effectiveness of treatment plan with documentation of patient care outcomes.Revises and modifies the care plan as needed to achieve health and medical goals.Refers care to other health care professionals and community agencies as needed.Prescribes pharmacologic (including controlled substances) and non-pharmacologic agents.Communicates effectively with patients and family for understanding of diagnosis, treatment plan, and need for follow-up care.Provides education to patients, families or caregivers as needed.Documents according to established guidelines to facilitate ongoing quality care in a timely manner.Responds in a timely manner to phone calls regarding patient needs.Contributes to effective and efficient practice management policies and practices that support patient access to medical care.Participates in access planning for applicable area or population.Supports appropriate billing for professional service through timely documentation and maintaining adequate knowledge of FVC billing policies and procedures.Adheres to all relevant policies, procedures, and practice guidelines.Assists with the preparation for JCAHO and other regulatory surveys.Manages patient care through appropriate use of available resources.Appropriately prescribes and orders ancillary services.Utilizes Fairview referral network appropriately.Manages chronic health/disease conditions.Provides leadership for clinical and support staff.Serves as a resource to support staff, enhancing quality of care.Participates in the training, orientation, and evaluation of clinical and support staff.Participates in appropriate clinical investigation relevant to specific patient population.Responds to nursing staff educational needs in formal and informal venue demonstrating nursing procedures, charting procedures, and reviewing pathophysiology of disease and new treatment modalities.Serves as consultant and role model for nursing staff.Maintains competence and professional development. Engages in continuing education.Is active in professional and other appropriate organizations.Demonstrates behaviors that support performance improvement activities.Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.Community Information:Bass Lake providers are connected to the larger M Health Fairview network, including specialty care, community hospitals, and the University of Minnesota academic health system.
