The Home Health Speech Therapist in the Northern AZ territory provides specialized speech therapy services primarily to adult patients within their homes. This role involves developing and implementing individualized treatment plans to improve communication, swallowing, and cognitive-communication abilities. As a key member of a multidisciplinary team, the therapist collaborates closely with other healthcare professionals and reports to the clinical manager. Minimal travel within the service area is required, and the therapist will utilize electronic health records to document patient progress and care plans.

Responsibilities

• Conduct patient assessments to determine speech therapy needs
• Develop individualized treatment plans tailored to patient goals
• Implement speech therapy techniques to improve communication and swallowing
• Provide patient and family education on therapy goals and strategies
• Document patient progress using electronic health records
• Coordinate care with multidisciplinary team members
• Evaluate patient safety and recommend necessary equipment
• Perform follow-up visits to monitor and adjust treatments

Preferred Qualifications

• Minimum 1 year of experience in speech therapy
• Valid state license in Speech-Language Pathology
• Strong skills in patient assessment and treatment planning
• Expertise in speech therapy techniques and patient education
• Proficient documentation and record-keeping abilities
• Excellent communication and time management skills
• Demonstrated empathy and patient-centered approach
• Experience working within a multidisciplinary team
