Treatment & Triage Nurse

The Treatment and Triage Nurse provides direct patient care to adults with complex, chronic disease in an Ambulatory ICU Patient Centered Medical Home setting. The role functions as a part of a large, integrated care team alongside the Primary Care Provider, Social Work, Nurse Care Management, Behavioral Health and others with the goal of providing whole-person care in the outpatient setting in order to avoid Emergency Room and hospital utilization by supporting our members to overcome their multiple barriers to care and self-management. Education & Experience Registered Nurse with 5 + years related experience in hospital or ED care of complex adult patients with multiple diseases Skilled with IV placement and phlebotomy Knowledge and experience using Electronic Medical Records and ability to analyze and leverage their reporting capabilities #J-18808-Ljbffr AbsoluteCare
