Position Summary

The MDS Coordinator assures that the federally mandated RAI (Resident Assessment Instrument) process is completed accurately and timely for the state of Nebraska. This position plays a critical role in maximizing the highest potential reimbursement for the financial viability of the facility, development of the total plan of care for each resident and the survey process, ensuring compliance with state and federal regulations.

Core Responsibilities
  • Exhibit behaviors that are consistent with the mission, vision, values, culture, and objectives of Gordon Memorial Hospital District.
  • Assess all resident cares, needs, and problems including but not limited to visiting with family members and the resident, reviewing current/ past records, and reviewing various data collection tools.
  • Conducts psychotropic monitoring, completes skin and physical assessments, and interviews caregivers to determine care needs.
  • Follow specific guidelines and time frames set up by the state of Nebraska to ensure appropriate classification and payment. This includes meeting the requests of various state departments, and submitting correction, termination, and significant change documents when appropriate.
  • Facilitate the involvement of appropriate health professionals needed to improve or maintain the residents' functional abilities at the highest practicable level.
  • Responsible for the completion of the entire RAI process (assessments, MDS, RAP's, care plan, RN Coordinator signature for completion) in accordance with the state and federal regulations.
  • Assures accurate data entry of RAI information into the Accumed system.
  • Maintain and periodically update written policies and procedures that govern the development use, and implementation of the resident assessment (MDS) and care plan.
  • Demonstrate effective communication and leadership skills.
  • All other duties as assigned.
Scope of Impact

The primary purpose of the MDS Coordinator is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of the Federal and state regulations as well as Company policy and procedures.

Qualifications
Position Qualifications
  • Education: Bachelor's degree in nursing or related field preferred.
  • Experience: Previous experience as an MDS Coordinator in a similar role within a long-term care setting preferred.
  • Licensure and Certification:
    • Current LPN or RN license in state of Nebraska required.
    • BLS certification required.
  • Knowledge, Skills, and Abilities:
    • Knowledge of:
      • Strong knowledge of MDS assessment process.
      • Medicare guidelines.
      • Medical technology.
      • Basic computer programs, i.e. Windows, Point Click Care, etc.
    • Skills in:
      • Exhibiting good nursing judgement and decision making.
      • Strong assessment skills.
      • Organization skills.
    • Ability to:
      • Visual and auditory ability to assess resident conditions.
      • Verbally communicate and problem solve effectively with residents, staff, physicians, and families.
      • Interpret medical records.
      • Complete forms and records accurately.
      • Prioritize workload.
