We offer flexible schedules, travel reimbursement, competitive health benefits, and continuing education benefits.Team 2: Atlantic Beach, Baldwin, Cedarhurst, East Rockaway, Bellmore, Freeport, Hewlett, Inwood, Island Park, Lawrence, Levittown, Lido Beach, Long Beach, Lynbrook, Merrick, North Bellmore, Oceanside, Point Lookout, Rockville Centre, Seaford, Valley Stream, Wantagh, & Woodmere.8 hour shifts (8:30AM to 4:30PM) - Rotating weekendsPer visits are required to attend an entire first week of orientation Monday – Friday (8:30AM -4:30PM.)Per visits are required to work 2 shifts per pay period.Job DetailsOur Home Care RN's:Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.Measure vital signs, administer IVs, change dressings, clean wounds as neededProvides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.Provide care to the most upmost standardMinimum Qualifications:New York State RN licenseMinimum of one year of recent hands on experience as a Registered Nurse.Basic Life Support CertificationValid New York State Driver's License and reliable transportationPosted Salary RangeUSD $35.00 - USD $135.00 /Hr.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
