Unit Information

Join our team as a Case Manager in a vibrant care coordination environment. The Case Manager will handle an average of 15-20 patient consultations per day in an acute care setting.

Key Responsibilities

Conduct initial assessments and discharge planning.
Manage utilization review processes.
Collaborate with various payers, including uninsured and homeless populations.
Utilize Interqual criteria for review and approval.
Participate in effective communication and teamwork within the case management team.
Adapt and reprioritize tasks throughout the day based on evolving patient needs.

Qualifications

Current BLS certification required.
Recent experience with Dignity Health in California is essential.
Proficient in Cerner charting system.
Strong communication and collaboration skills.

Additional Information

This position requires flexibility to work every other weekend and may involve holidays if the candidate is willing. Training in Interqual will be provided.

Work Environment

The role encompasses both utilization review and discharge planning within a supportive team that values effective outcomes and patient-centered care.
