Key Responsibilities\n

• Conduct thorough and comprehensive assessments of clients, identifying their social, emotional, and environmental challenges that may hinder their well-being.

• Develop tailored service plans, including specific goals and interventions, and monitor progress to ensure that clients are receiving the support they need.

• Provide individual and family counseling, guiding clients through their challenges, and helping them access necessary resources.
\n
• Engage in crisis intervention, performing risk assessments and creating safety plans as required to protect vulnerable clients.

• Collaborate with local community agencies and organizations, facilitating service coordination and advocating on behalf of clients to navigate social systems effectively.

• Participate in community outreach efforts to raise awareness of our services and foster collaboration across organizations.

• Attend case conferences and team meetings to contribute insights and coordinate strategies for client care.

Patient Population / Client Focus\n

Your primary focus will be on working with vulnerable populations in the Athens area. Clients may include individuals and families facing domestic instability, mental health challenges, and other socio-economic barriers. • Master's degree in Social Work from an accredited institution.
