Key Responsibilities\n

As an LMSW, your primary responsibilities will encompass a wide range of duties aimed at providing comprehensive support to patients within the hospice setting. Your daily tasks will include:

• Conducting thorough clinical assessments to identify the psychosocial needs of patients and families, developing tailored treatment plans that address their unique challenges.

• Implementing a variety of evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy, to support mental health needs and encourage coping strategies.

• Maintaining accurate and up-to-date clinical records in compliance with HIPAA regulations, ensuring confidentiality and quality of care.

• Coordinating with the interdisciplinary treatment team, including medical professionals, to optimize patient care and create holistic support plans.

• Facilitating educational sessions and providing resources to patients and families about available community support services and mental health intervention options.

Patient Population / Client Focus\n

Your work will primarily focus on individuals facing terminal illnesses and end-of-life issues. You will provide emotional support and navigate the complex dynamics of grief, loss, and family dynamics. By developing strong therapeutic relationships, you'll help empower patients to express their feelings and foster meaningful conversations about their care preferences and legacy.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid LMSW license in the state of New York.

• A minimum of 3 years of experience in clinical social work, with a focus on hospice, palliative, or end-of-life care settings.

Compensation and Benefits\n

The salary for this position ranges from USD 80,000 to 100,000 per year, commensurate with experience and qualifications.
