Work Environment\n

This position operates remotely, providing flexibility to ensure accessibility and responsiveness to client needs. While the bulk of your work will be conducted from a home office, you will be required to make regular visits to our partnered hospice facility in Camden, NJ, to assess client needs and ensure all care plans are being effectively executed. This role will demand effective time management and the ability to adapt to various environments based on client needs.

Key Responsibilities\n

As a Licensed Master Social Worker at Healing Arts Social Work Services, you will:

• Conduct comprehensive psychosocial assessments that evaluate emotional, mental, and social factors affecting seniors.

• Collaborate closely with clients and their families to create individualized care plans focused on their specific objectives.

• Provide counseling and emotional support, guiding families and seniors through transitions such as end-of-life care, health crises, and major life changes.

• Advocate for client rights by assisting them to access community resources and ensuring they receive the necessary support services.

• Engage with interdisciplinary teams—including nurses, caregivers, and other healthcare professionals—to coordinate holistic care that addresses the comprehensive needs of clients.

• Regularly visit clients to monitor their well-being, ensuring that their living conditions are adequate and addressing any concerns that may arise.

• Facilitate communication among family members to resolve conflicts and promote understanding surrounding care decisions.

• Educate clients and their families about community resources, support groups, and social engagement opportunities to enhance their overall quality of life.

• Maintain precise and detailed documentation of all assessments, care plans, progress notes, and client interactions in accordance with ethical standards and organizational policies.

• Stay compliant with social work policies and maintain knowledge of current best practices within the field.

This position primarily focuses on seniors who are receiving hospice care, along with their families. Working with this demographic requires an understanding of geriatric issues, palliative care principles, and the emotional complexity that often accompanies end-of-life services. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
