Key Responsibilities\n

• Conduct thorough assessments of patients’ psychosocial needs upon admission to determine appropriate intervention strategies.

• Develop individualized treatment plans that are tailored to the unique circumstances and preferences of each patient and their families.

• Utilize evidence-based solution-focused therapies to address issues such as grief, anxiety, and depression.

• Facilitate both individual and family therapy sessions, focusing on improving coping mechanisms during transitional moments.

• Document all interactions and progress accurately and confidentially in our electronic health record system.

• Engage collaboratively with an interdisciplinary treatment team, including physicians, nurses, and chaplains, to ensure holistic care.

• Participate in ongoing training and professional development to stay abreast of best practices and changes in mental health regulations.

Our Hospice Center serves a diverse patient population, including individuals facing terminal illness and their families. Qualifications\n

• A Master’s Degree in Social Work from an accredited university.

• Active and unrestricted LMSW license in the state of New Jersey.

Preferred Experience\n
