Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, incorporating emotional, mental, and social factors.

• Collaboratively develop personalized care plans with clients and their families, addressing their specific goals and needs.

• Provide counseling and emotional support to seniors, guiding them through life transitions such as health scares, loss of independence, or family changes.

• Advocate for client rights, ensuring they have full access to community resources and necessary services.
\n
• Team up with other professionals, including healthcare providers and caregivers, to ensure seamless coordination of care.

• Regularly engage in virtual visits to monitor client well-being, analyze living situations, and address potential safety concerns.

• Facilitate communication among family members, resolving any conflicts regarding the client’s care strategies.
\n
• Educate clients and their families about available local resources, support groups, and activities that promote engagement and social interaction.

• Maintain detailed documentation of assessments, care plans, and progress notes, ensuring compliance with ethical standards.

• Stay current with social work policies, practices, and developments that impact the care of seniors.

Patient Population / Client Focus\n

In this role, you will focus primarily on the elderly population, addressing a wide range of issues prevalent in later life. This includes mental health challenges such as depression and anxiety, social isolation, health-related concerns, and family dynamics. You will play a critical role in not just improving the quality of life for these clients but also promoting their overall emotional and psychological well-being.

Qualifications\n

• A Master's Degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

Compensation and Benefits\n
