Work Environment\n

This position is primarily remote, allowing for flexibility and work-life balance. However, the Social Worker will coordinate with clients and their families through virtual consultations and occasional in-person visits as deemed necessary. Candidates must be adaptable, as there will be times when on-call availability is required to address urgent client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, evaluating emotional, mental, and social factors that contribute to their overall health and well-being.

• Collaborate closely with clients and their families to develop individualized care plans that reflect their unique goals, preferences, and challenges.

• Provide counseling services and emotional support, assisting seniors and their families as they navigate significant life transitions, including health crises, relocation, or bereavement.

• Advocate for clients’ rights by ensuring access to community resources and social services, facilitating their engagement with treatment and support systems.

• Work collaboratively with an interdisciplinary team that includes healthcare providers, caregivers, and nurses to coordinate care and monitor client progress.

• Engage in regular client visits to assess their living conditions, well-being, and to address any emerging needs or safety concerns.

• Facilitate conflict resolution and open communication among family members involved in the elder care process, fostering a supportive environment.

• Educate clients and families about community programs, support groups, and resources available to enhance their social engagement and mental health.

• Maintain meticulous documentation of client interactions, assessments, care plans, and progress notes to ensure continuity of care.

• Stay updated with the latest developments in social work practices and ethical standards to provide the highest quality service to our clients.

Patient Population / Client Focus\n

The primary focus of this role is on seniors, many of whom face complex health and social issues. As a Social Worker at Serene Reflections Therapy, you will be working with clients who may be experiencing challenges related to aging, chronic illness, mental health concerns, or family dynamics. Your expertise will empower clients to improve their quality of life and ensure they receive necessary support and services.

Qualifications\n

• A Master’s degree in Social Work from an accredited educational institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience working in a social work capacity, particularly within community health settings or with geriatric populations.

Compensation and Benefits\n

We offer a competitive salary ranging from $60,000 to $100,000 per year, commensurate with experience and qualifications.
