Work Environment\n

This position offers a remote work setting, allowing flexibility while maintaining a strong connection to our community. You will primarily engage with clients virtually, but will also have opportunities to make in-person visits as needed. Our support staff will assist in coordinating resources and scheduling, ensuring you have the tools and support necessary to succeed. Working hours are from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend availability required based on client needs.

Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the emotional, mental, and social needs of senior clients.

• Collaborate closely with clients and their families to create personalized care plans that align with their unique goals.

• Provide empathetic counseling and emotional support, assisting clients in navigating significant life transitions and challenges.

• Advocate for clients’ rights and facilitate access to essential community resources, ensuring comprehensive support.

• Resolve conflicts and foster communication among family members to support the client's care and well-being.

• Educate clients and families about community resources and social engagement programs to enrich their quality of life.

• Maintain up-to-date documentation of all client interactions, assessments, and progress notes utilizing our electronic health record (EHR) system.

• Stay updated on the latest social work practices and regulations to ensure compliance with ethical standards.

Your primary focus will be on seniors, a demographic that often faces unique psychological and social challenges. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

Compensation and Benefits\n
