This position operates within a remote work setting, allowing for a flexible schedule conducive to your lifestyle. Full-time work is typically conducted Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as client needs demand. The role necessitates adaptability, as you may be required to be on-call at times to address urgent situations effectively and ensure continuity of care for our clients.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of seniors facing health challenges, ensuring a thorough understanding of their emotional, mental, and social needs.

• Develop and implement individualized care plans in partnership with clients and their families, aligning with their preferences, goals, and available resources.

• Provide empathetic counseling and emotional support to clients and their families to help them navigate complex life transitions and challenges associated with palliative care.

• Act as an advocate for clients, helping them gain access to vital community resources, services, and support needed for their well-being.

• Collaborate with interdisciplinary teams, including nurses, healthcare providers, and caregivers, to coordinate comprehensive care and enhance client outcomes.

• Conduct regular check-ins and visits to monitor clients' conditions, address any concerns regarding their living situations, and ensure safety.
