Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observations, and review of medical records to identify the emotional, social, and environmental factors impacting patients' recovery.

• Provide empathetic counseling and support to patients and their families, addressing adjustment challenges and emotional well-being.

• Collaborate closely with the rehabilitation team to design and implement individualized treatment plans that accommodate patients' psychosocial needs.
\n
• Assist patients in connecting with community resources, including support groups, vocational rehabilitation, and housing assistance, ensuring all options are accessible and understood.

• Educate patients and families on available community resources and post-rehabilitation services to enhance their transitional experience.
\n
• Participate in discharge planning and facilitate effective transitions for patients returning home or moving to alternative care settings.

Patient Population / Client Focus\n

In this role, you will work with a diverse patient population primarily consisting of individuals in hospice care, addressing complex health-related psychosocial issues. Your focus will be on facilitating emotional healing, providing guidance during critical transitions, and enhancing patients' quality of life during their rehabilitation processes.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• A strong understanding of psychosocial issues, including adjustment to serious illness, grief, and effective coping strategies.

• Exceptional communication and interpersonal skills to build rapport with patients, families, and team members.
